Gold Thread-Qualitative: (Evaluation, Treatment Plan, Services provided, Progress Notes, Treatment Plan Review, Discharge Planning)

“Gold Thread” is a term coined by mental health consultant David Lloyd, of MTM Services. It refers to the concept that risk free and clinically sound documentation in a medical chart demonstrates that each service ordered on the treatment plan comes from a need identified in the diagnostic assessment; that each service, including frequency and duration, is medically (clinically) necessary to improve the client’s functioning; and that the progress notes chart directly to the plan of care, justifying the charge to the payer. Anyone who audits for Gold Thread should be terminally licensed in his/her field meeting the requirements of Mental Health Professional.

Gold Thread consistency establishes a legal justification for the services provided. Medical necessity should drive every treatment decision from intake to discharge.

Value: Something a policy document asks the organization for, such as the title of its clinical director, shown in the library as a name in double brackets, like [[CLINICAL_DIRECTOR]]. A value not yet answered appears in the document that way.

Version of a Standard: One revision of a standard, which takes effect on a date. The Joint Commission and CARF call their versions editions. An organization using CARF works from the edition it chooses (see CARF Editions). See Question Sets and Versions of a Standard.
